Description

You will be a driven and motivated indiviudal, capable of working on your own initiative to develop and build our new Hospital Discharge Service. This is a vital service to help people to leave hospital and live independently at home for longer. You will manage staff to deliver excellent outcomes to people and be able to demonstrate your empathy to this cause.

Base Salary
£28,269-£32,682
Responsibilities
  • To develop and manage the Hospital Discharge Service, including recruitment and management of staff.
  • To form positive relationships with commissioners and others, understanding the requirements to win work for the Hospital Discharge Service.
  • To provide a range of services to support the Health and Wellbeing Manager including:
  • Assist with the implementation of quality procedures including health and safety, incident reporting and risk assessments.
  • Ensuring the staff are performing to expectations through auditing and monitoring
  • Assist with the marketing of services particularly on social media and the website
  • Compile monitoring forms and annual surveys as needed
  • Organise a programme of staff training
  • Produce welcome packs for clients, as required
  • Utilise the online accounting system (Xero)
  • Use the CRM system to update staff and client information, rota’s and attendances checking accuracy prior to invoicing and payroll
  • Ensure appropriate staffing levels by effectively managing and authorising absences and overtime
  • Attend and present at external events and meetings to raise the profile of the centre
  • To perform other tasks allocated as appropriate
Qualifications
  • Commitment to relevant training and development
  • Strong organisational skills and excellent time management skills
  • Good communication skills
  • The ability to work well as part of a team as well as working individually
  • Computer literate and a thorough knowledge of Microsoft Office products
  • A high level of English spelling and grammar
  • Accuracy and attention to detail

  • Flexibility as well as a professional approach
  • The ability to work on own initiative
  • The ability to adapt to the changing needs of the service, clients and staff.
  • The drive to forge relationships with clients, partners and carers so that complete trust and understanding is achieved.
  • Full Driving Licence (Over 21 for Insurance to drive company vehicles)
